What happens to your feedback?

Information submitted through this form will be reviewed by the CMSE DEI Committees, and may be used to inform efforts to support and improve the department. Please include your contact information if you would like a direct response. 

Can I send anonymous feedback?

Yes! We will not actively seek to identify individuals who submit feedback without contact information. However, please remember that we are a small department and it may be possible to recognize individuals inadvertently based on details they might share. For example, we have just 1 or 2 individuals from certain countries, so you might accidentally identify yourself if you include specific information about your nationality.

What should NOT be reported here?

This is an informal feedback mechanism, not a means of providing an official report to the University. There is information on the Resources and Education pages about how to report violations of University policy (including concerns related to RVSM or discrimination), potential crimes, or threats to health or safety. Please note that if we receive information about potential policy violations or other significant concerns through this form, we may be required to report to the appropriate University officials (even if the feedback is submitted anonymously).
