Form to request enrollment in a class that is full or restricted. This is used to get access to restricted "Special topics in CMSE" courses (CMSE 491 or 890), and also by undergraduate students who wish to enroll in 800 and 900-level CMSE courses.  Emails from course instructor and/or graduate director will be accepted in lieu of a signature.

Required for students wishing to apply for a dual PhD in CMSE and a second discipline (regardless of whether CMSE is the primary or secondary discipline).  Carefully read the dual PhD information page to understand the requirements for this program - CMSE will not approve your dual PhD if you do not completely fulfill the requirements!

CMSE PhD students must bring this form to their comprehensive exam (their first "official" dissertation committee meeting) and have their committee sign off on it.  It must be turned into the CMSE graduate secretary along with a digital copy of your comprehensive exam paper.

This form is the reporting form associated with the annual dissertation committee meeting for CMSE PhD students (and dual PhD students primarily in CMSE).  It is used for all committee meetings except for the first one (which is the comprehensive exam record form) and the final meeting (which is the dissertation completion form).

Form to request an independent study with a CMSE faculty member (CMSE 499 or 891).  This is NOT required to take a restricted "Special topics in CMSE" course; that is the course override request form.

Graduate students pursuing the CMSE Graduate Certificate in Computational Modeling must fill out this form to enroll in the graduate certificate program.  The purpose of enrollment is to ensure that you are fulfilling the requirements for the certificate and so that we can make you aware of opportunities available to students in CMSE graduate programs.
